Change of Address Form

Businesses and individuals which have been issued licenses, permits, registrations, certifications, or any other requirement by New York City may need to re-apply or renew these documents when they change locations or change their official mailing address. The process for updating a business mailing address may vary by issuing agency. Businesses may be required to submit a renewal form, a change of address form, or to re-apply for a given license or permit.
Holders of licenses, permits, registrations, or certifications issued by New York City should contact the issuing agency about the process for changing their mailing address.
